OVERVIEW


Care Coordination for Children with Special Health Care Needs

 
The Caring Program helps children with chronic conditions and special health care needs get the care they need. By providing family-centered, community- based care coordination, we believe we can maximize the quality of life for these children and their families. This program is free for children with special health care needs who are already enrolled in Pennsylvania's Children’s Health Insurance Program (CHIP) through Highmark CHIP benefits.
 

The Caring Program currently provides the following services for children enrolled in CHIP through Highmark:

 

 

Comprehensive Case Management

 
Comprehensive Case management is when clinical staff and providers work hand-in-hand with a child and their family or caregivers. We provide the personalized care your child needs to create and monitor a plan to help improve their quality of life, achieve health goals and minimize costs.

ELIGIBILITY

The Caring Program is available free of charge to children enrolled in Pennsylvania’s Children’s Health Insurance Program (CHIP) through Highmark in Highmark's service area. Families’ CHIP benefits or eligibility are NOT impacted by participation in the Caring Program.
 
More than 21,000 families have benefited from the Caring Program since its inception in early 2005.

Children with chronic conditions and/or special health care needs often need certain accommodations to address their specialized needs. A number of resources are available to help children, families and educators make the necessary adaptations that can enable the child to receive the best education possible.
