What do I do if I lost or want to change my password or username?

Visit the website and click Log In. Then click Forgot username or password? Follow the steps shown.

How do I change the font size on my member website?

Click the font size icon at the top of any page on the website. Choose the size you want.

How can I see a copy of my ID card, fax it to my doctor or order a new one now?

Log in to your member website and click ID Card on the home page. View your ID card or click on the links to fax, print or order a new one. You can also order a new ID card by phone. Just call 1-800-241-5704.

CLAIMS AND SPENDING

Where can I get information about a claim?

Click the Claims tab on your member website. Go to Claims & Payments to see:

  • Any claims from the past 2 years
  • Doctor and facility charges
  • What your plan paid
  • Claim status or what you owe

To see claim details just click See More on the claim you want to review.

What’s an Explanation of Benefits (EOB)?

An Explanation of Benefits (EOB) explains how your benefits have been applied. It shows what you may owe after your health insurance claim has been processed. You can also see how much you have paid toward your deductible. You should review it to make sure you received the services that were billed.

How can I see my EOBs?

Click the Claims tab on your member website. Then go to Claims & Payments. Find the claim you want to review. Click See More to find the link to download your EOB.

Want to receive EOBs by email? Here’s what to do. Go to your member site and click Your Account. Then click Account Settings. Go to Contact Preferences and select Go Paperless.

What is a Plan Activity Statement?

This is a monthly report for members who have spending accounts. It shows recent account activities and a year-to-date summary.

How can I see my Plan Activity Statement?

Click the Claims tab on your member website. Then go to Claims & Payments. Click See More on any claim. Then click the link to download your Plan Activity Statement.

Want to receive Plan Activity Statements by email? Here’s what to do. Go to your member site and click Your Account. Then click Account Settings. Go to Contact Preferences and select Go Paperless.
